DSMES 7 Health Habits Certification (Employer Training Program)

Our DSMES 7 Health Habits Certification equips your CHWs, nurses, and allied staff with the clinical, educational, and operational skills needed to deliver high-quality diabetes education—while supporting sustainable, billable services for your organization.

Designed for hospitals, clinics, FQHCs, and community-based health systems, this training goes beyond theory to support real-world implementation.

Major Operational Advantage: Billable Patient Education

DSMES services are billable under Medicare and many payers when delivered by trained staff within an accredited program.

  • Up to 10 initial billable hours (1 individual, 9 group) within the first 12 months

  • 2 billable follow-up hours annually in subsequent years

  • Billed using HCPCS G-codes (G0108 individual, G0109 group)

  • Requires physician referral, proper documentation, and program alignment

Most programs are designed to be self-paced or completed in structured cohorts over 4–12 weeks, depending on the program and employer needs.

A high school diploma or equivalent, access to a laptop or desktop computer, and a stable internet connection. Our team can assist with onboarding and tech support.

We currently offer certifications in DSMES (Diabetes Self-Management Education and Support) and Community Health Worker (CHW) training. CHW learners can also pursue a Department of Labor-recognized credential, if desired.
